What should I do about a problem like this, which still isn't fixed after a month? I reported the issue via the form that shows up after failed resolution attempt. Also contacted email address in your post.
I continue to find non-resolving DOIs as part of a project I'm working on - but I unfortunately have little confidence that reporting them will do me any good.

Thanks so much for taking the time to write and to report broken DOI links to CrossRef.
We too are very concerned when CrossRef DOIs are publicized before the publisher has deposited them at CrossRef. We continue to exhort our publishers to make sure this doesn't happen (See our most recent newsletter for an example. http://www.crossref.org/10quarterly/quarterly.html#report6)
Because we have a very small staff (about 10 people for a volume of more than 40 million DOIs), unfortunately we do not have the ability to respond personally to each problem report for DOIs. However, please be assured we do investigate them all and report each of them to the responsible publisher when appropriate.
We completely agree that making DOIs publically available before they have been activated is an unacceptable process, and we will continue to do what we can to keep it from happening.
The best way to help is to continue doing what you are doing, which is to report broken CrossRef DOIs on this form: http://www.crossref.org/DOIComplaint/. In the case that you mention where a CrossRef DOI worked but then stopped working, the circumstances of the link would be very helpful.
